If you have a progressive illness, a time may come when you’re no longer able to make decisions or give direction concerning your health care. Advance directives give you the opportunity to communicate your preferences for your future care, or to designate a person who will be legally responsible for making decisions about your care.

If you feel strongly about life support, resuscitation measures, or any other type of treatment, filing these forms can help you rest assured that your wishes will be carried out.
