Physicians and nurses from Methodist Health System are volunteering their time to help athletes get a start on the upcoming sports season. We're teaming up with athletic district officials to offer sports physicals and echocardiograms.

Forms:

Parents of middle school and high school student athletes who wish to participate in the echocardiogram screening must accompany their athlete. Parents must complete a medical history questionnaire with signed medical releases prior to the screening.
