Phase II Cardiac Rehabilitation at Methodist Dallas

Phase II cardiac rehabilitation is an early outpatient program designed to assist patients to return to normal activities as soon as possible following a heart attack or cardiovascular procedure.

Candidates for Phase II cardiac rehabilitation include patients who are post- or have a diagnosis of

*Medicare approved diagnosis
Most private insurance companies provide coverage for these Medicare approved diagnoses and others such as heart failure – however, this varies from carrier to carrier. Verification of coverage prior to program initiation is completed by the Folsom Fitness and Rehabilitation Center staff.

Phase II cardiac rehabilitation at the Folsom Fitness and Rehabilitation CenterThe Phase II cardiac rehabilitation at the Folsom Fitness and Rehabilitation Center, offered three days a week, consists of:

  • Continuous telemetry monitoring and physician/medical supervision
  • Baseline and ongoing patient assessment
  • Risk factor management
  • Exercise/activity training three times a week
  • Group education
  • Individual counseling as needed
  • Program lasts two to 12 weeks or 6 to 36 sessions
  • Physician referral required

Individualized personal and program goals are established with each patient upon entry into the program.
