PGY2 Transitions of Care Residency

PGY2 Transitions of Care Residency Program at Methodist Dallas Medical Center

The post graduate year two (PGY2) program is designed to equip the resident with the skills necessary to function as the primary transitions of care pharmacist by managing all aspects of medication therapy in both inpatient and ambulatory care settings. The emphasis will be placed in providing excellent pharmaceutical care in conjunction with the multidisciplinary team. The resident will gain experience in transitioning the patient from the inpatient to outpatient setting for a core set of disease states/patient populations including heart failure, COPD, stroke, diabetes, MI, and solid organ transplant. Teaching activities include didactic presentations, involvement with UNTHSC School of Pharmacy and Pharmaceutical Sciences courses, and clerkship pharmacy students in their clinical rotation. The resident will also be involved in a major research project.

The 12-month residency program consists of the following rotations:

Ambulatory Care Clinic
Inpatient Internal Medicine
Solid Organ Transplant
Medication Therapy Management
Emergency Medicine
Infectious Diseases
Methodist Patient Centered Accountable Care Organization (MPCACO)
Pharmacy Administration
Weekend Staffing

o Current curriculum vitae
o Letter of intent
o Three references
o An official copy of the college of pharmacy transcript

Requirements for Acceptance

• Pharmacy degree from an accredited college of pharmacy
• Graduation from an ASHP accredited PGY1 program
• Eligibility for employment at Methodist Dallas Medical Center
• Participation in the residency matching program
• On-site interview
